VC runtime and patching

I have an application that used vc8.1
When i installed it, i choose the vc8.1 runtime.
Since then i have upgraded to vc9
I need to build a patch that will upgrade from the vc8.1 runtime to the vc9 runtime.
I noticed that the runtime pre-requisites section in the msi code was commented out for patches. So i removed the comments. When i installed it ran the check, told me i needed the runtime, then started to run the vc9 msi file.
But it failed. I ran the same msi on the machine and it worked fine. So evidently the vc9 runtime is not being included in the patch.
How do i do this. I need to upgrade the runtime in the patch?
Can i tell installaware to put the vc msi code in the patch somewhere. I i need to install it and and run it manually afterwards.
