Patch work if done soon after first installtion but not...

Hi all,
Please can anyone give us a pointer in the right direction.
We have an issue with a patch program that we have created.
We are using InstallAware version 8 (are now bying v9 to see if it works better), admin rights etc..
Now to the issue:
If we have version 1 and what to patch to version 1.1 and you do the patching more or less directly after the first version has been installed we have a 100% success rate. However if we do the patch a few month or weeks later sometime the patch is not installed (without any warnings what so ever).
Does anyone know if this is a bug or are we doing something wrong?
Please any help would be greatly appreciated, we have been trying to solve this now for months...?
