Selecting a Path for SQL Database Installation

I am trying to setup a dialog for selecting a path to locate my SQL database files. I have tried the "destination" dialog but this gives a pre-formed paths name that I don't want. Does anyone have an idea on this.

P.S. I would also like a progress bar to show while creating the database tables.
