I tried that same installer (demo.exe) on several Windows 7 computers on our domain and it wouldn't pin on any of them, nor did it show the pin options on the shortcut. Interestingly enough, I tried it on Windows 8 and it worked fine. With UAC off it still did not work on Windows 7 so I'm guessing at this point it may have something to do with security software or something along those lines as that is the only common application on the tested Windows 7 computers. Even run as Administrator, it still does not work, so it's possible the security software or domain settings are preventing the creation of the pin option.
I'll have to do further testing and will let you know what I find so you can pass it along to future customers.

I tried that version already as well. I think it must have something to do with policy settings on the domain.
I removed all security and rebooted, still didn't work.
I installed it on the server and the pin options showed up fine and it pinned to the taskbar.
However, on any Win 7 workstation it won't pin the first shortcut, only the second. I would bet at this point it's a permission/policy issue of some sort.....no idea what the solution is though. Will have to go through trial and error to figure it out.

I found the solution to this problem after many hours of troubleshooting.
If you have a project named 'Test Demo' and another project named 'Test', and each project has 2 shorcuts (e.g. Test and Other) you cannot install the Test project after Test Demo project on the same computer otherwise pinning will fail on the first shortcut and the pin to taskbar options will not work. You have to delete the first instance of the registry entry named 'test' under applications in the registry to fix it, otherwise pinning is permanently broken.
The solution is just to name your projects with unique names if there is a space between the project names (e.g. ESILAW Demo and ESILAW Newsite, not ESILAW and ESILAW Demo). Registry seems to get buggered up if you do this and pinning no longer works on any future installations with those names.

I have the same problem. I have 3 applications:
"app"
"app pro"
"app pro x64"
If I install "app pro" and "app pro x64" I always get only one pinned shortcut, but never two, if I install only one of them they always have their shortcut pinned.
If I install "app" and "app pro" or "app pro x64" the shortcut of ecvery application is pinned to taskbar.
I do not understand why this happens, also I tried the approach of michaels but without success.
