Folder selection from Command Button

I added a command button to a form, it selects a custom folder when clicked, how do I get the selected value from the pop up folder selector into a text box or variable?

I have tried every way to Sunday and I must be missing something terribly simple.

There's a built-in theme you can inspect to see how this is done - open the destination dialog from the Plain theme.
